Livestock Market Holding PVP Sale

 

1.   Producer must submit Pre-Conditioning Form to sale administrator (Livestock market,
      TLN Executive Director Ben Neale or TDA Animal Health Tech) prior to sale.

2.  VIP Coordinator will then mail the producer
     a.  Grant Authorization and Payment Form
     b.  Substitute W-9 to be completed and signed

3.  Producer must return forms back to the VIP Coordinator for payment to be issued

     P.O. Box 40627, Nashville, TN 37204
