Re-Certification Form

Re-Certification with the Governor’s Office of Diversity Business Enterprise is currently required every two (2) years from the original certification date. To re-certify, please complete the “Affidavit of No Change Form” and provide a copy of the most recent federal tax return including all schedules.

The “Affidavit of No Change Form – PDF” document confirms your company’s status as a diversity business and also enables you to update any changes of ownership, company contact information, etc.

Firms that provided an existing certification at the time of their initial registration by a certification agency, ex: Tennessee Department of Transportation (TDOT), Women Business Enterprise National Council (WBENC) U. S. Small Business Administration, State Purchasing Councils, Uniform certification agencies, etc., must provide a copy of their current certification at the time of renewal.

Note: Firms that have allowed their certification with Go-DBE to lapse or expire must complete a new Certification application in order to renew their certification with Go-DBE.
