This vendor registration form is available for your use. Simply type in the required information, save the form to your hard drive, and then mail a printed copy (along with all requested documentation) back to the Governor's Office of Diversity Business Enterprise.

It is important to note that the registration form does not complete the process. An appointment to further discuss any assistance, procurement interests, qualifications, etc. will be confirmed during the assessment.
