Duplicate of License/Name & Address Change

This application is provided to make changes to personal information on your Tennessee teaching license and/or request a duplicate copy of your license. Download Application for Duplicate License or Amendment to File Form pdf or call (615) 532-4885 to request the form be mailed to you.

An Address Change
Answer the legal questions, sign, date the application, complete Part I and Part II, and mail to the address provided at the bottom of the application.

A Duplicate of License
Answer the legal questions, sign, date the application, complete Part I, Part II, Part III, Part IV, sign, date, and mail to the address provided at the bottom of the application form. THIS APPLICATION MUST BE NOTARIZED

A Name Change
Answer the legal questions, sign, date the application, complete Part I, Part II, Part III, Part IV, sign, date, and mail to the address provided at the bottom of the application. You must also attach legal documentation supporting the name change. (Options of legal verification accepted by the Office of Teacher Licensing are: a photocopy of the notarized or certified marriage license, or divorce decree or the court order that has generated the legal change of name) THIS APPLICATION MUST BE NOTARIZED

NOTE: If you have an expired license, no changes can be made to your license. The Office of Teacher Licensing can not provide you with a copy of an expired license. A notarized letter of verification will be provided.
