The Release of Funds form on the website at http://state.tn.us/education/fedprog/fpforms.shtml was to be submitted by March 31^st, 2008 for the FY07 (SY06-07) Title projects. For accountability purposes, this original signed form must be received by Betty Cochran from all LEAs, whether they have funds to release or not. A green field indicates she received the original signed form; a gold field indicates she received a fax copy, but no original was received. A red field indicates no form was received. Those LEAs with either a gold or red field must submit an original signed copy for the field color to be changed to green.
