LEAs that have Title I High Priority schools and are
required to offer Supplemental Educational Services (SES) must submit all
required reports. This includes SES evaluation and monitoring data, as follows:
v
CREP surveys
(principals, teachers);
v
CREP Student Level
Achievement database for SES statistical analyses (student’s name, SSN,
provider, system, school, demographics, etc.);
v
SES Provider
information to SDE (number of students, amount paid, total hours); and
v
Local Consolidated
Application budget information for PSC and SES (to be checked during team
reading).
Submission dates will vary, but all information must be
submitted no later than June 1, 2008.
