Provide the full legal name of your organization, the mailing address of your organization as recognized by the U.S. Postal Service (including the Zip Code), the street or location address if different from mailing address, the business telephone number and business fax number. Include the county in which your organization is located.

Check the appropriate box which describes your organization. If you are unable to determine which status to check, please call this office for assistance. If you have been designated a Service Educational Activity (SEA) by the Department of Defense, sign Part III and go to Authorized Representatives, Page 2 form.
If your status is a non-profit, tax-exempt organization, you must provide all documents listed in II A., B., C., and D. Failure to do so will result in your application not being processed.
In addition, please note special documentation requirements for the following organizations:
Programs for Older Individuals: Must include evidence of funding under the Older Americans Act of 1965, Titles IV or XX of the Social Security Act, Titles VIII or X of the Economic Development Act of 1964, or the Community Services Block Grant Act.
Programs for Assistance to the Homeless: Must include a letter from the mayor, county judge, city or county health officer or comparable authority that certifies that applicant is a "provider of assistance to the homeless." Included in the letter must be an identification of the service or assistance provided.
Programs for Assistance to the Impoverished: Must include proof of how financial eligibility is determined for program recipients. A letter is required from agency head that oversees social programs in the local jurisdiction (i.e. County Director of Social Services) indicating that services provided are provided primarily to the impoverished. Must attach charter, bylays or Articles of Incorporation, and any program literature.
Rescue and Volunteer Fire Organizations: Must include a letter from appropriate county official that rescue organization is funded wholly or in part by tax dollars. If not supported wholly or in part by tax dollars, organization cannot be eligible as an organization apart from the local government and must acquire property through the local government. Rescue organizations must also include a copy of charter of incorporation and a copy of the organization's membership certificate in the Tennessee Association of Rescue Squads.
Museum: Must provide proof that organization employs a full-time curator, and is open to the general public for specified set hours.
Library: Must furnish proof that library is open to the general public.

The organization's head official (County Executive, Mayor, School Superintendent, Highway Superintendent, President, Executive Director, Chief, etcetera) must provide an original signature. Photocopy, rubber stamp, or machine produced signatures will not be accepted.

The organization's head official should designate those persons to represent his organization to acquire surplus property. To designate each person, the person's typed or printed name should be in the first column, the job title of the designated person in the second column, an original signature of the designated person, and the Tennessee driver's license number of that person. Failure to completely provide requested information will result in your application being returned. Complete the purchasing information as requested, and provide an original signature of the organization's head official.
