State of Tennessee The two elevator service contracts awarded, effective November 30, 2005, have been amended, for the provision of full preventive maintenance, inspection, adjustment, replacement and repair services of elevator equipment in State owned buildings, universities, and colleges described in Attachment 1 of the contracts. Contract attachment 1 lists the buildings, elevators, monthly maintenance cost per elevator, and the facility point of contact and telephone number. Elevators noted in attachment 1 in BOLD font are new additions and deletions.
Downtown Nashville Group-Manned Sites
Contract FA-06-16631-00 covers State facilities described in Attachment 1 that are identified as the Downtown Nashville Group-Manned Sites.
Unmanned Sites
Contract FA-06-16630-00 covers State facilities described in Attachment 1 that are identified as Unmanned Sites.
Billing information is contained in Section C.3, Payment Methodology, of the contracts. Preventive and Complete Maintenance - The Payment Methodology section, Section C.3., contains monthly preventive maintenance billable cost information by elevator site group. Contract attachment 1 provides the billable cost information by individual elevator. Agencies desiring to establish a spending authority against these contracts in STARS should forward a request to Ron Plumb, Department of General Services, Ron.Plumb@state.tn.us. The request should include appropriate fund, allotment code, object code, cost center and amount. Agencies that do not use STARS, such as State Board of Regents schools, should utilize an appropriate procedure to establish spending authority. The Contractor will submit monthly invoices to facility points of contact. Facilities under the administrative control of the Department of General Services shall verify receipt of service and forward invoices to the State point of contact listed below for payment processing. Other agencies should coordinate invoicing procedures through the Contractor. Upgrade of Equipment and Work Performed Outside the Scope of Preventive and C omplete Maintenance - The Payment Methodology section contains information on labor rates and parts mark-up percentage, for upgrade services and work considered to be other than preventive and complete maintenance work. Such work requested by the State shall be performed by the Contractor at the labor and parts rates quoted in the contract. No upgrade or work outside the scope of preventive and complete maintenance shall be performed without the prior authorization of the State. The column in Contract Attachment 1 titled “Upgrade Not to Exceed Cost” is FOR INFORMATION AND PLANNING PURPOSES ONLY. Agencies desiring to proceed with elevator upgrade projects are responsible for identifying funding and for following applicable procurement procedures. Upgrade projects for facilities under the control of the Department of General Services will be coordinated through the Division of Property Services Management.
