Living Well with Chronic Conditions
A Six-Week Workshop
Living Well with Chronic Conditions (the Chronic Disease Self-Management Program or CDSMP) is a six-week workshop that provides tools for living a healthy life with chronic health conditions, including diabetes, arthritis, asthma and heart disease. Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with the emotions that chronic conditions may bring about.
Chronic Disease Self-Management programs supported by the Tennessee Department of Health include the Living Well with Chronic Conditions workshop and the Diabetes Self-Management Program, a workshop for people with diabetes.
Living Well workshops are now available in many counties in Tennessee.
There are more than 70 Living Well Leaders in Tennessee and more than 10 Master Trainers.
All Leaders and Master Trainers must complete a full 4-day training to become certified as a Leader.
Participants must attend the full training and facilitate a program to become a Leader.
Two trained Leaders are required to lead chronic disease self-management programs.
The Tennessee Living Well Program has a variety of resources for local Living Well programs, including an Implementation Guide, marketing and recruitment resources, quality assurance and fidelity monitoring tools, presentations, and data collection forms.
