Register for the Blind
T.C.A. 49-10-804 mandates that the Department of Human Services maintain a register of citizens who are legally blind and living in Tennessee. Ophthalmologists, optometrists, and other medical professionals are required by this law to report to the Register the names and pertinent facts of any person who is examined and diagnosed to be legally blind. In addition, all individual citizens, organizations, agencies and professional persons are encouraged to submit to the Register the names of any legally blind persons they may know. All information provided to the Register is confidential and is used only for purposes specifically related to the administration of the program. Information needed to complete a referral to the Register includes the individual’s name, address, telephone number, social security number, date of birth, gender, visual acuity, and the cause of the vision loss.
