Health Institute Treatment Planning DepartmentThe Treatment Planning Department's primary purpose is to ensure the timely development of high-quality treatment plans that meet the individual needs of service recipients. The planning of service recipients' treatment requires an excellent interdisciplinary team process, accurate documentation consisting of assessments, treatment plans, progress notes, Psychosocial Rehabilitation (PSR) group participation; and the implementation and revision of the individualized interventions outlined in the plan. The treatment planning Facilitators are responsible for facilitating the treatment team’s progress towards all three of these requirements. The Facilitators coordinate meeting schedules and document the team's decisions on the Master Treatment Plans and Treatment Plan Reviews. The Facilitators act as a liaison with the staff of the PSR to coordinate changes to objectives, interventions, and group schedules so that group leaders and teams have accurate and updated information. In addition, the department uses performance improvement measures to develop high quality standards for treatment planning. Treatment Planning Facilitators lead PSR groups that orient service recipients to the treatment planning process. The Facilitators also assist service recipients in processing their concerns and issues. |
