Supportive Living Housing Program

Service Description
TN Code Annotated §12-4-330 directs the Department to reimburse certain supportive living facilities in 11 Tennessee counties. The Fiscal Services Section administers this program. The rate of reimbursement is $2.00 a day x 30% of eligible residents.

This service provides a supplement to eligible facilities in certain counties in Tennessee. After a facility is certified by the Fiscal Services Section to be eligible for the program, facilities submit to the Section each month reimbursement forms. The Section reimburses these claims until the funds for the program run out.

Why do we fund it?
The program is funded because of state law (see above). Most of these facilities are underfunded and the program does provide some supplement to these facilities.

Whom does it serve?
The program serves adults who are diagnosed with mental illness and co-occurring disorders who are residing in supportive living facilities eligible according to TN Code Annotated § 12-4-330.

What are the outcomes?
The program reduces the likelihood of hospitalization and the use of acute care, increases community tenure and the likelihood of employment, ensures that consumers receive the needed services in order to successfully integrate into the community, improves the quality of life, and reduces the reliance upon more costly services. The amount of funding is too small to make a measurable difference in a facility. A home can usually expect no more than $2,000 a year from this program
