Intensive Long-term Support

Service Description
Intensive long-term, wrap-around support services include psychiatric, nursing, case management and treatment services, designed to allow discharges of service recipients from Moccasin Ben Mental Health Institute (MBMHI) and to maintain the service recipients in the community in supportive living facilities, which complement existing services funded by various departments of the state (case management, outpatient psychiatric services, mobile crisis services, peer support centers, etc), which have not been able sufficiently to meet the specialized needs of this population. Housing support, on-site psychiatric services, case management, nursing services, day program and other needed services are provided via contracts with Volunteer and Fortwood Center.

Why do we fund it?
The Intensive Long-term Support Project was implemented to reduce the census at MBMHI in order to allow for the closing of the Winston Building. This component of this project is designed to allow the successful discharge of service recipients from MBMHI who would not be able to be discharged without this level of treatment and support services.

Whom does it serve?
The program serves adults who are diagnosed with mental illness and co-occurring disorders and who are discharged from MBMHI.

What are the outcomes?
The program reduces the likelihood of hospitalization and the use of acute care, increases community tenure and the likelihood of employment, ensures that consumers receive the needed services in order to successfully integrate into the community, improves the quality of life, and reduces the reliance upon more costly services. Individuals are discharged from the Institute who could not live outside the institutional setting otherwise due to a lack of essential, customized, wrap-around services. Reports indicate that the individuals are successfully being integrated into the community and that recidivism rates are low. Evaluations of 32 of the participants enrolled in the programs yielded the following results. Before receiving services in Fortwood the individuals, during a period of 52,727 cumulative days spent a total of 17,955 days (34%) institutionalized using inpatient services at MBMHI and 37,772 days (66%) living in the community. Since being placed in the program, during a period of 14,098 cumulative days, they spent only 1,480 days (10%) at MBMHI and 12,618 days (90%) in the community.
