IMPORTANT: Beginning January 15, 2008, the state will begin a verification process for re-enrollment into the CoverRx program. If you are in the Mental Health Safety Net and would like to enroll in CoverRx, please complete the application and fax or mail it to CoverRx at any time. ( http://state.tn.us/mental/recovery/CoverRxForm.pdf ).Be sure to sign the application where indicated and include your income and the number in your household.

Beginning January 1, 2007, Tennessee launched CoverRx, the state's new pharmacy assistance program. The program is designed for low-income individuals at or below 250 percent of the Federal poverty level in addition to other eligibility criteria. Benefits include access to approximately 250 medications with a sliding-scale co-pay.
