Apply for a Disabled Person License Plate/Placard

Disabled Driver PlateTo apply for a disabled person license plate or parking placard, the applicant must be supported with a certification from one of the following:

  • a medical doctor licensed to practice medicine,
  • a physician's assistant or nurse practitioner acting in conjunction with a written protocol developed jointly by a physician,
  • a Christian Science Practitioner listed in the Christian Science Journal

The certification is included on the Application for Disabled Person License Plate/Placard (Form RV-F1310301) and must include the following:

  • Any mechanical devices used, i.e., crutches, braces, etc.
  • Whether or not applicant is permanently confined to a wheelchair
  • The nature of the disability (see Reference Material for Disabled Person License Plate/Placard Application Section 1
  • If the disability is permanent or temporary
  • The name, full address and telephone number of the certifying medical professional
  • The signature of the certifying medical professional
  • Date of certification's completion
