If you have a complaint against an institution, you should try to resolve that complaint directly with the institution. If you are unable to resolve the dispute with the institution, you can file a complaint with the Department. To file the complaint electronically, please proceed to the next step. However, please note the following: When briefly stating your complaint, please refrain from using degrading language. Please fill out all of the fields on the form that apply to your complaint.  Please print a copy of the complaint form for your records prior to submitting your complaint electronically. Please be advised that with the submission of this form you authorize this Department to conduct an investigation to determine if a violation of Tennessee law has occurred.
