2009 Templates

The following templates can be used to help run your Summer Reading Program. It is the responsibility of the library's to print the reading logs, so choose the log(s) that are most appropriate for your library. • Full Page Completion Form (.pdf format)
• Volunteer Form (.pdf format)
• Black & White CSLP Completion Certificate (.pdf format)
• Reading Log Numbered Titles - (.pdf format) - This reading log is a full-page, folded reading log, similar to past year's logs, with lines available for 45 titles.
• Reading Log Numbered Titles & Time - (.pdf format) - This reading log is similar to previous year's. It is one page, folded, with room for 45 titles AND time.
• Half Page Reading Log - (.pdf format) - This is a half page reading log, double-sided reading log with space for 15 titles.
• Half Page Reading Log Including Time- (.pdf format) - This Reading Log is half-page, double-sided, and has room for 15 titles AND time.
• Pre-Reader Reading Log Game - (.pdf format) - This Reading Log is designed for PreReaders. As they complete tasks on the squares, they are to cross them off until they have a diagonal, vertical, or horizontal line. These logs are half-page, double-sided and there are unnumbered lines on the back to list titles read.
• Ages 5 - 12 Pre-Reading Log Game - (.pdf format) - This Reading Log is designed for Ages 5-12. As they complete tasks on the squares, they are to cross them off until they have a diagonal, vertical, or horizontal line. These logs are half-page, double-sided and there are unnumbered lines on the back to list titles read.
• Teens Reading Log Game - (.pdf format) - This Reading Log is designed for Teens. As they complete tasks on the squares, they are to cross them off until they have a diagonal, vertical, or horizontal line. These logs are half-page, double-sided and there are unnumbered lines on the back to list titles read.
• Adults Reading Log Game - (.pdf format) - This Reading Log is designed for Adults. As they complete tasks on the squares, they are to cross them off until they have a diagonal, vertical, or horizontal line.
